Does a Negative-Dilute DOT Drug Test Require a SAP in South Carolina?

No. A negative-dilute DOT urine drug test, by itself, does not require a Substance Abuse Professional (SAP) evaluation. South Carolina CDL drivers should distinguish that result from a verified positive test or a refusal, which can require the SAP return-to-duty process.

The distinction matters when a carrier asks you to return to a collection site: another collection is not automatically a referral to a SAP. This guide concerns federally regulated DOT testing, not a company's separate non-DOT testing program.

What does negative-dilute mean?

Negative describes the drug-test result. Dilute describes a urine specimen with creatinine and specific gravity below expected levels. These are different findings; the word dilute does not turn a negative result into a positive result.

Will I have to take another DOT test?

Sometimes. Under 49 CFR 40.197, an MRO-directed recollection is mandatory and directly observed when creatinine is at least 2 but no more than 5 mg/dL. For other negative-dilute results above 5 mg/dL, the employer may require recollection under its consistently applied policy. Those recollections are not directly observed unless another regulatory basis requires observation.

The recollection result becomes the result of record. Declining a directed recollection is a refusal. Ask the employer's designated employer representative (DER) or the medical review officer (MRO) to clarify the instructions you received; do not assume a negative-dilute result lets you disregard them.

Is a negative-dilute result a Clearinghouse violation?

FMCSA says an MRO should not report a negative-dilute result to the Clearinghouse. Its guidance separately explains that a negative-dilute return-to-duty result is reported by the employer or C/TPA as a negative return-to-duty test.

That reporting distinction does not erase an existing violation or replace the steps required for someone who is already prohibited from driving. Do not confuse a new test result with confirmation that an earlier return-to-duty process is complete.

When is a SAP evaluation actually required?

DOT requires a SAP evaluation after a DOT drug or alcohol regulation violation before the employee resumes DOT safety-sensitive duties. Examples include a verified positive drug test, an alcohol result of 0.04 or greater, or a refusal to test. A positive-dilute result is treated as a verified positive, not as a negative-dilute result.

When the SAP process is required, the SAP conducts an individualized evaluation, determines the required education and/or treatment, and makes the referral. The client does not select or prescribe that requirement.

What should a South Carolina driver clarify first?

Whether you are applying for port-related work in Charleston or a trucking position in the Upstate, start with the actual reported result rather than the shorthand phrase "failed test." Useful questions for the DER or MRO are: Was the result negative-dilute or verified positive? Is another collection required? What instructions must I follow?

These questions clarify the testing record; they do not ask you to diagnose a specimen result or decide your own SAP requirements. This article does not determine your employment eligibility or current Clearinghouse status.
