Service Description

The $400 SAP fee includes the initial SAP evaluation, follow-up SAP evaluation, and required FMCSA Clearinghouse reporting. Educational and/or treatment recommendations are separate from the SAP fee and may involve additional costs. Important: During checkout, you may see "Duration: 30 Days" or similar language. This is a system-generated checkout setting and does not indicate a recurring charge or a required 30-day program. Your $400 SAP fee is a one-time payment and covers the SAP services included in your package. After checkout, you will receive intake documents through SimplePractice. SimplePractice will also be used for secure messaging, appointment reminders, and your virtual SAP evaluations. To make getting started easier, eligible clients may choose to pay over time using Affirm, Klarna, or Afterpay at checkout.
