Before Your DOT SAP Evaluation: What Drivers Should Prepare
A DOT drug or alcohol violation can create pressure quickly. Drivers often want to know what to do first, what information they need, and how to avoid slowing down the return-to-duty process. Preparing before the initial SAP evaluation can make the appointment more focused and help the process move more smoothly.

Confirm your FMCSA Clearinghouse designation
For FMCSA-regulated drivers, the SAP process cannot move correctly until the driver selects the SAP in the FMCSA Drug and Alcohol Clearinghouse. This designation allows the SAP to complete required reporting once the evaluation and compliance steps are finished.
Before your appointment, make sure you can log in to your Clearinghouse account and confirm that the correct SAP has been selected. If you are unsure how to do this, ask for guidance before the appointment instead of waiting until after the evaluation.
Complete intake paperwork promptly
SAP appointments require paperwork, consent forms, and identifying information. Completing these forms early helps avoid last-minute delays. It also gives the SAP enough information to understand the violation, employment context, and any history that may be relevant to the recommendation.
Drivers should use accurate contact information and answer questions honestly. Missing or incomplete paperwork can delay scheduling, documentation, and next steps.
Gather basic details about the violation
You do not need to prepare a legal argument for the SAP evaluation, but you should be ready to discuss what happened. Useful details may include the date of the violation, type of violation, employer or prospective employer information, testing circumstances, and any previous DOT drug or alcohol violations.
The SAP may also ask about substance use history, treatment history, recovery supports, work history, current stressors, and safety concerns. The purpose is to make an appropriate DOT-compliant recommendation, not to shame the driver.
Have a valid photo ID ready for telehealth
Three Corners Counseling provides virtual DOT SAP evaluations for eligible drivers who are physically located in North Carolina or South Carolina at the time of the appointment. A valid photo ID is required. Drivers should also use a private location, stable internet connection, and a device with working audio and video.
A quiet and private setting matters because the evaluation includes confidential clinical and compliance-related information. Driving, multitasking, or meeting from a public place can interfere with the appointment.
Understand that the recommendation comes after the evaluation
Many drivers want to know before the appointment whether they will need education, treatment, or another step. The SAP recommendation is made after the evaluation, based on DOT requirements and the clinical information reviewed during the appointment.
After the initial evaluation, the driver must complete the recommendation before the follow-up SAP evaluation can occur. Once the driver successfully complies, the SAP can update the Clearinghouse with the required completion information. The employer or prospective employer then handles the return-to-duty test and follow-up testing plan.
